The challenge
Arbor's intake form was a twelve-page PDF. Staff re-keyed every submission by hand, and roughly a third arrived incomplete.
Our approach
We rebuilt intake as a progressive mobile flow with save-and-resume, then wired it directly into the practice management system so nothing needs re-keying.
The outcome
Intake completes before the appointment starts. Front-desk staff moved from data entry to actual patient support.
